Coincidentally, I learned today about a TV show coming out that has a story line fairly identical to one in one of my books. I'd be pleasantly surprised if it had been stolen. For one thing, it would mean I had sold more books.

Not happened to me, but I've heard of it happening.
Like people above, I (and plenty of people I know) have had books sold on sites like Amazon (but not amazon), by someone who downloaded them from Amazon and then uploaded them to the other site(s). Seems a common way to profit off other writer's works. They were charging more than I do, too. I got those I found removed. Had quite a few shorts I've written on sites like reddit published on sites without people asking for permission too, but I don't have much of a problem with it as I appreciate that's a risk of putting something online publicly. Plus if anyone likes them, hopefully they look up the author to read more (plus they weren't very edited or very good and I wasn't going to do anything with them).

Selling a pirated copy is one thing, but slightly reworking someones book or short story and claiming it as their own is just blatant, at least a pirated copy still has the authors name attached, even though they don't get paid for it.

Yeah, there are literally hundreds of facebook pages sharing illegal pdfs of books. Publishers employ people full-time whose job it is to track down sites, FB pages etc and get them to take stuff down. Which they do. And then they just change name and start all over again....

No, but I've found books I had on Amazon on pirate sites. C&Ds are almost not worth the effort because there are so many of them. I don't think anything I have "indie" is worth plagiarizing, but if you epub it's almost impossible to avoid getting pirated.
